This study demonstrates that bile extracellular vesicles can serve as a novel liquid biopsy source for protein biomarkers that enable early detection of cholangiocarcinoma in PSC patients. Given the 15–20% lifetime CCA risk in PSC and absence of effective surveillance tools, this represents a high-impact early detection approach.
