This narrative review synthesizes preclinical and early clinical evidence on thymic regeneration strategies targeting immunosenescence, including GH/IGF-1 axis modulation, IL-7/KGF cytokines, mTOR inhibitors, sex steroid ablation, mesenchymal stem cell transplantation, and gene therapy. Multiple approaches have active clinical trials supporting thymic biology as an actionable longevity target with potential to restore T-cell naivety and reduce inflammaging.
